Alternate Planes of existence are the homes of the Gods. These are collectively referred to as the Planes of Power, not to be confused with the expansion of the same name. The Planes Norrathians see are actually the physical manifestation of the Gods' realms within Norrath's Plane of existence; as such, the Gods found in these planes are just the projection of the actual God. Once "killed" the God's power is diminished and takes time to rebuild at which point a new physical projection of the God comes into being.
There are several ways to access these planes including teleportation through an Alter Plane spell and finding a corporeal portal.
[edit] Planes and Demi-Planes
So what's the difference between a Plane and a Demi-Plane? It's quite simple really. A Plane (or Full-Plane) is the home of a Greater God. These are the realms that have always existed as have their Deities. A Demi-Plane is the home of a Demi-God. A Demi-God is one who was created by a Greater God (excluding The Nameless who created the Greater Gods), as such, a Demi-Plane is a realm of existence that was created to house the Demi-God. Also referred to as "Prime Deities" and "Sub-Deities."
Example: Cazic Thule resides in the Plane of Fear. At some point, he took a mortal and bestowed her with some of his power making her an immortal Demi-God Terris Thule. When he did this, he created a realm for her as an extension of the Plane of Fear. This became the Demi-Plane of Nightmares.

Some deities are referred to as Prime even though they are children of Prime deities, examples include Terris-Thule and Solusek Ro. Conflicting lore exists all over EQ.

[edit] The Realms of Order and Discord
There is some confusion among players as to whether or not the place in which the Omens of War expansion takes place is a plane or not. It is not a plane; it is a physical galaxy somewhere within the plane of existence that encompasses Norrath.
The Realms of Order and Discord are not physical places like the planes, but are actually spheres of influence. Rytan described this as overlapping spheres of opposing power. Norrath and Kuua are both located in the overlap area, sort of like being at the mid-point between two planets of equal side with gravity working in both directions. The spheres of influence are conceptual and not physical.
