This avatar of Terris Thule, appearing in the Plane of Time, is generally either the first or second mob that a raid kills in phase 4. Terris and Saryrn are the easier two mobs; Tallon and Vallon Zek are both much tougher.

Terris Thule in here is a relatively simple fight. She has two AoEs; Phantasmal Torment (mana drain/DoT), and Quivering Nightmares (hate increaser and stat reducer). Neither is particularly devastating; the mana drain puts a sort of time limit on the fight, but it's not terrible.

The real meat of the fight happens in the adds. Every 20% (confirmation needed) of her health, she spawns four adds. The adds cannot be mesmerized and must be killed.
